- If the subscriber no longer meets the requirements for getting the Lifeline discount or stops participating in at least one of the state or federal programs required to do so, they promise to tell Cintex Wireless as soon as possible and no later than within 30 days.
- The subscriber understands that it is illegal to use false or fraudulent information to obtain Lifeline assistance.
- The subscriber is aware that it might be necessary to recertify continuous eligibility for Lifeline at any time, and that if it doesn’t do so within 30 days, it will be disenrolled from the program and lose access to its benefits.
- Under penalty of perjury, the subscriber requesting Cintex Wireless Lifeline service certifies that all of the aforementioned statements made when requesting service are accurate and true to the best of the subscriber’s knowledge.

A Social Security Number may be requested from a subscriber when applying for service in order to confirm their eligibility for Lifeline service.
